Moorfields Patient Health Coaching Service at City Road

Moorfields Health Coaching Service is a free and confidential service available to Moorfields patients under the External Disease and Glaucoma Teams care. Our trained Health Coaches offer personalised support to help you feel more in control of your health—whether you're adjusting to sight loss, managing long-term conditions, or simply looking to improve your lifestyle.

Our Health Coaches have experience in sight loss and understand the unique challenges it can bring. They’re here to listen, guide, and empower you to make changes that matter to you.

What can a Health Coach Help With?

  • Managing sight loss alongside other health conditions
  • Building confidence and motivation
  • Improving sleep, nutrition, and physical activity
  • Reducing stress and boosting emotional wellbeing
  • Setting realistic goals and making lasting changes
  • Navigating health services and other support options

How does it work?

  • Coaching sessions may be one to one or in groups – are usually 45-50 minutes long and we start by offering 6 sessions.
  • Sessions are tailored to your needs and goals and agreed at the start of coaching.
  • Sessions may be delivered in person, via video or phone and are available Monday to Friday 9-5 pm.
